Job summary

Castle Rock Home Care Inc. seeks a motivated Home Care Field Marketer to develop and maintain referral sources across New York City.

You will build relationships with hospitals, social workers, discharge planners, and community groups to drive patient referrals for home care services. The role requires knowledge of NYC Medicaid/MLTC, a field-first mindset, and a track record of meeting referral targets in a fast-paced environment.

Qualifications

• 1+ year of healthcare marketing or medical sales experience.
• NYC experience strongly preferred.
• Existing relationships with MLTC plans, hospitals, or nursing facilities is a major plus.
• Knowledge of Medicaid home care services and referral processes in NYC.
• Strong interpersonal and networking skills.
• Self-motivated with the ability to work independently in the field.

Responsibilities

• Develop and maintain relationships with referral sources including hospitals, rehab centers, nursing homes, and social workers
• Promote home care services to discharge planners, case managers, and community organizations
• Educate referral partners on services offered through Licensed Home Care Services Agencies (LHCSAs)
• Generate and track new patient referrals and admissions
• Conduct in-person visits across NYC boroughs (Brooklyn, Queens, Bronx, Manhattan, Staten Island)
• Represent the agency at community events, health fairs, and senior centers
• Collaborate with intake and coordination teams to ensure smooth patient onboarding
• Maintain accurate records of outreach activities and referral pipelines
• Ensure HIPAA and NYS Department of Health regulations compliance

The ideal candidate understands the NYC healthcare landscape, including Medicaid and Managed Long-Term Care plans, and is comfortable working in a fast-paced, target-driven environment.

• Develop and maintain relationships with referral sources including hospitals, rehab centers, nursing homes, and social workers

Compensation & Benefits

• Pay: 65,000 - 125,000 a year
• Performance-based bonuses/commissions (based on referrals/admissions)
