Job summary

Castle Rock Home Care Inc. is seeking a motivated Home Care Field Marketer to develop and maintain referral sources across New York City, focusing on hospitals, clinics, and social workers. The role requires 1+ year in healthcare marketing or medical sales, NYC familiarity, and the ability to travel across NYC boroughs for outreach events and in-person visits.

Qualifications

• High school diploma required; Associate’s or Bachelor’s degree preferred.
• 1+ year of experience in healthcare marketing, home care, or medical sales (NYC experience strongly preferred).
• Existing relationships with MLTC plans, hospitals, or nursing facilities is a major plus.

Responsibilities

• Develop and maintain relationships with referral sources including hospitals, rehab centers, nursing homes, and social workers.
• Promote home care services to discharge planners, case managers, and community organizations.
• Educate referral partners on services offered through Licensed Home Care Services Agencies (LHCSAs).
• Generate and track new patient referrals and admissions.
• Conduct in-person visits across NYC boroughs (Brooklyn, Queens, Bronx, Manhattan, Staten Island).
• Represent the agency at community events, health fairs, and senior centers.
• Collaborate with intake and coordination teams to ensure smooth patient onboarding.
• Maintain accurate records of outreach activities and referral pipelines.
• Ensure all marketing practices comply with HIPAA and New York State Department of Health regulations.

The ideal candidate understands the NYC healthcare landscape, including Medicaid and Managed Long-Term Care plans, and is comfortable working in a fast-paced, target-driven environment.
