Open Sky Community Services is seeking Outreach Counselors in Central Massachusetts to coordinate treatment with local agencies and families. You will deliver services under supervision, help clients engage in community activities, and support independent living goals.

The role requires a high school diploma or GED, a valid driver’s license, and a reliable vehicle. On-call rotations are part of the duties, with a focus on respectful, person-centered care.

Qualifikationen

• High School Diploma or GED or equivalent is required.
• Valid Driver’s License and reliable insured vehicle with acceptable driving record are required.
• Reliability, willingness to learn and openness to new opportunities are essential attributes.

Aufgaben

• Deliver interventions identified on the Treatment Plan with supervision from licensed clinical staff.
• Implement activities that develop valued roles and personal relationships in the community.
• Provide side-by-side support and coaching to help individuals socialize.
• Conduct 1:1 and group activities to increase readiness for rehabilitation.
• Assist with obtaining financial support by helping find gainful employment or apply for entitlements.
• Participate in the On-Call rotation.
