Job summary

Open Sky Community Services in Massachusetts is seeking an Outreach Counselor to coordinate treatment, support clients in building social and vocational skills, and connect them with community resources.

The role emphasizes delivering evidence-based interventions under supervision, assisting with employment/entitlements, and participating in on-call rotations. A valid driver’s license is required as is a high school diploma or GED.

Qualifications

• High School Diploma, GED or equivalent, required.
• Valid Driver’s License, reliable and insured vehicle, and acceptable driving record, required.
• Reliability, willingness to learn, and being open to new opportunities.

Responsibilities

• Deliver interventions identified on the Treatment Plan with supervision from licensed clinical staff.
• Implement activities that create opportunities for development of valued roles and personal relationships in the community.
• Provide side-by-side support and coaching to help individuals socialize; conduct 1:1 and group activities to increase readiness for rehabilitation.
• Assist with finding gainful employment or applying for entitlements to ensure adequate financial support.
• Participate in the On-Call rotation.
