Job summary

Open Sky Community Services seeks an Outreach Counselor to coordinate treatment and support for adults with mental health needs in Central Massachusetts. You will liaise with clients, families, and community agencies to implement the treatment plan.

You will provide direct social coaching, arrange employment and entitlements, participate in on‑call rotations, and help individuals build independent living skills.

Qualifications

• High School Diploma or GED or equivalent.
• Valid Driver’s License, reliable and insured vehicle, and acceptable driving record.
• Reliability and willingness to learn; open to new opportunities.

Responsibilities

• Deliver interventions identified on the Treatment Plan with supervision from licensed clinical staff.
• Implement activities that create opportunities for the development of valued roles and personal relationships in the community.
• Provide side‑by‑side support and coaching to help individuals socialize. Conduct both 1:1 and group activities geared toward increasing Persons’ readiness to engage in rehabilitation.
• Ensure individuals have adequate financial support through assisting with finding gainful employment or applying for entitlements.
• Participate in the On‑Call rotation.
