Open Sky Community Services is seeking an Outreach Counselor in Massachusetts to deliver interventions, support social development, and assist with employment and entitlements. The role emphasizes community integration and trauma-aware, person-centered practice within a multidisciplinary team.

You will work under supervision from licensed clinical staff, participate in on-call rotations, and contribute to helping individuals pursue meaningful goals while navigating resources and supports in the

• High School Diploma, GED or equivalent, required.
• Valid Driver's License, reliable and insured vehicle, and acceptable driving record, required.
• Reliability, willingness to learn, and being open to new opportunities.

Responsabilidades

• Deliver interventions identified on the Treatment Plan with supervision from licensed clinical staff.
• Implement activities that create opportunities for the development of valued roles and personal relationships in the community.
• Provide side-by-side support and coaching to help individuals socialize.
• Conduct both 1:1 and group activities geared toward increasing Persons' readiness to engage in rehabilitation.
• Ensure individuals have adequate financial support through assisting with finding gainful employment or applying for entitlements.
• Participate in the On-Call rotation.
