Job summary

Open Sky Community Services is seeking an Outreach Counselor to connect individuals with community resources, employment, and education while supporting recovery and independence in everyday life.

The role involves coordinating with families, agencies, and clinics, delivering plan-based interventions, and leading 1:1 and group activities to promote social engagement and skill-building.

Qualifications

• High School Diploma or GED or equivalent.
• Valid Driver’s License with reliable car and acceptable driving record.
• Reliability and willingness to learn new opportunities.

Responsibilities

• Deliver interventions identified on the Treatment Plan with supervision from licensed clinical staff.
• Implement activities that create opportunities for valued roles and personal relationships in the community.
• Provide side-by-side support and coaching to help individuals socialize; conduct 1:1 and group activities to increase readiness for rehabilitation.
• Assist individuals with securing financial support through employment or entitlements.
• Participate in the On-Call rotation.
