Job summary

Open Sky Community Services seeks an Outreach Counselor to coordinate treatment with partners, families, and community services. You will support individuals to engage in rehabilitation, link them to employment resources, and participate in on-call rotations, under supervision of licensed staff.

Qualifications

• High School Diploma, GED or equivalent required.
• Valid Driver’s License with reliable, insured vehicle.
• Reliable, willing to learn and open to new opportunities.

Responsibilities

• Deliver interventions identified on the Treatment Plan with supervision from licensed clinical staff.
• Implement activities that create opportunities for the development of valued roles and personal relationships in the community.
• Provide side-by-side support and coaching to help individuals socialize.
• Conduct 1:1 and group activities geared toward increasing Persons’ readiness to engage in rehabilitation.
• Assist with finding gainful employment or applying for entitlements to ensure financial support.
• Participate in the On-Call rotation.
