Job summary

Endeavors is seeking an Intake Coordinator/Outreach professional in Tucson, AZ to support CBP employees and eligible family members through outreach and eligibility screenings. The role emphasizes data management, referral coordination, and collaboration with Case Manager to maximize program impact.

Qualifications include a Bachelor's degree or equivalent experience and at least one year in case management and outreach.

Qualifications

Bachelor's Degree in related field preferred, or equivalent related experience.
Minimum one year of Case Management experience.
Minimum one year in outreach and marketing activities preferred.
Experience with first responders or law enforcement is strongly preferred.
Ability to lead and foster teamwork and a supportive environment.

Responsibilities

Set up initial client file for CBP clients and their eligible family members entering the program.
Assign and schedule clients to health and wellness programs and track progress.
Prepare and keep data, files, reports, drafts, and accurate reporting of intake/referral/list activities.
Collect data for monthly, quarterly, and annual reports.
Assist in monitoring program referrals, intakes, and wait lists and report on objectives.
Manage daily assignments and ensure deadlines are met.
Report outreach efforts weekly and collaborate to improve processes.
Demonstrate exceptional customer service aligned to mission.
