Job summary

Catholic Community Services of Southern Arizona is seeking a PATH program support specialist to facilitate check-ins with participants and coordinate referrals to case managers for enrollment and follow-up.

In this role you will maintain participant documentation, manage caseloads, assist with complex cases, and track biopsychosocial data and service plans within HMIS. The job may be demanding and require steady performance, with occasional driving for agency business.

Qualifications

• Strong communication skills, oral and written
• Experience documenting participant information and case notes
• Ability to obtain First Aid and CPR certification

Responsibilities

• Facilitates program participant check-ins
• Connects with referrals; coordinates enrollment and follow-up with Case Managers
• Documents participant records and enters data into HMIS
• Carries a participant caseload and assists with complex cases
• Tracks biopsychosocial data and Individualized Service Plans
• Publishes daily participant bathing facility schedule and ensures facility sanitation
• May drive agency or personal vehicle on company business
