Job summary

EvergreenHealth is seeking an Outreach Coordinator to join the Community Healthcare Access Team (CHAT). The role focuses on helping patients navigate the healthcare system, connecting them with resources, and assisting with health insurance and other basic needs.

Based primarily at EvergreenHealth Kirkland, the coordinator will work with patients in ED, clinic, and community settings to improve access to care and services.

Qualifications

• Bachelor’s Degree required.
• Age-specific or behavior-specific knowledge and experience in working with specific patient populations relevant to area of assignment.
• Verbal and written language skills to effectively communicate, ensure, and provide for patient needs, and to provide a legal record of this care.
• Accountability for professional practice, commitment to patient care, and upholding the Patient Bill of Rights.

Responsibilities

• Screens and responds to incoming requests: Provides information about community resources, referrals, and assistance with accessing services; Screens requests for service; assesses to determine type and level of service; collaborates with the care team.
• Conducts initial assessments with clients and develops a plan for service or referral.
• Collaborates with various team members and links patients to primary or specialty care as appropriate.
• Provides office-based case management per guidelines for programs; refers patients to primary care and/or specialty care as appropriate.
• Identifies and conducts outreach activities within the community in collaboration with department management.
