Job summary

Volunteers of America Massachusetts is seeking a Community Support Navigator in a full-time, hybrid role to provide recovery support navigation and care coordination for clients with behavioral health needs. You will work across VOAMASS sites and community-based settings, travel as needed, and complete trainings on-site.

A Bachelor's degree and at least two years in social services are preferred; a valid driver's license and reliable transportation are required.

Qualifications

• Bachelor's Degree or related field preferred.
• Minimum of two years' experience with social service organizations and case management coordination.
• Valid driver's license and reliable transportation required.

Responsibilities

• Engage with persons served following admission to help individuals access treatment services within VOAMASS and externally with community providers.
• Meet with clients in the community or the office to identify needs, write service plans, and support action steps toward goals.
• Engage and build rapport with the person served, assist with referrals to services including housing, employment, and health supports.
• Provide follow-up support to ensure appointments are kept and goals are pursued.
• Establish and maintain relationships with referral providers to ensure care coordination.
• Coordinate care with prescribers, clinicians, VOA staff and external providers to support wrap-around services.
• Maintain records per VOAMASS policies, completing documentation promptly for payors and VOAMASS.
