Job summary

Volunteers of America of Massachusetts seeks a Community Support Navigator to provide recovery support navigation and care management for a high‑need caseload in a full‑time hybrid role.

The Navigator serves clients in the community, VOAMASS sites, and Behavioral Health clinics. A Bachelor’s degree is encouraged; two years of social service experience is preferred, with strong communication and a valid driver’s license. Training is provided on hire.

Qualifications

• Bachelor’s degree required or desirable in psychology, social work, health care administration, or related human service fields.
• Minimum two years’ experience with social service organizations, case management coordination, and service connection.
• Excellent communication, organization, and computer skills; valid driver’s license and reliable transportation.
• Knowledge of MA resources and system navigation is a plus.

Responsibilities

• Engage with persons served after admission to help access treatment services.
• Meet with clients in community or office to assess needs and write service plans.
• Support clients with referrals to services including housing, employment, and health care.
• Provide follow-up to ensure attendance and engagement in treatment.
• Maintain relationships with referral providers to coordinate CSP services.
• Coordinate care with prescribers, clinicians, and VOA staff to support wrap-around services.
• Maintain records and documentation per VOAMASS policies.
