Job summary

Open Sky Community Services is seeking a Community Services Navigator in Massachusetts. The role focuses on case management and outreach for individuals in Community Services Programs, Recovery Support Navigation, and related services.

You will develop Support Plans, coordinate with multiple providers, and assist with transportation during transitions to community resources; on-call rotation is required.

Qualifications

• High School Diploma, GED or equivalent, required.
• Valid Driver’s License with reliable transportation.
• Minimum of 2 years’ experience with homelessness, justice involvement, serious mental illness, substance use, or underserved populations.
• Excellent communication, documentation, time management and organizational skills.

Responsibilities

• Provide case management and direct outreach to assigned caseloads.
• Develop a Support Plan guiding services with other care plans, complete assessments on time.
• Coordinate treatment with community agencies, medical and behavioral health providers.
• Provide temporary transportation assistance as needed during transition to community resources.
• Participate in an On-Call rotation.

Individuals served in the program must have a mental health disorder or co-occurring disorder and be considered at high risk for admission to a 24-hour treatment setting.

Open Sky’s Community Supports Program is a Mass Health funded navigator service that provides case management and care coordination to individuals in the community. Individuals served in the program must have a mental health disorder or co-occurring disorder and be considered at high risk for admission to a 24-hour treatment setting.

The Community Services Navigator will provide case management and direct outreach support to their assigned caseloads, which consist of members enrolled in Community Services Programs, Recovery Support Navigation, Behavioral Health Justice Involved and/or Community Support Program for People Experiencing Chronic Homelessness.

Other Key Responsibilities

• Develop a Support Plan with the member that guides services in a way that is consistent and supportive of other existing comprehensive care plans. Complete all Assessments and Support Plans according to prescribed timelines and specifications.
• Act as a liaison between members, community agencies, and families to coordinate treatment, including employment and education entities, Care Coordination entities, Emergency Service Providers, outpatient clinics, and others. Work collaboratively with medical and behavioral health providers to obtain any active comprehensive care plans that the member may have.
• Provide temporary assistance with transportation to essential medical and behavioral health appointments while transitioning to community-based transportation resources.
• Participate in an On-Call rotation.

Qualifications

• High School Diploma, GED or equivalent, required.
• Valid Driver’s License with acceptable driving history and reliable, insured transportation.
• Minimum of least 2 years’ experience working with or lived experience with homelessness, justice involvement, adults with serious and persistent mental illness, substance use disorders, and underserved populations, required.
• Excellent communication, documentation, time management and organizational skills required.

Pay Range

$22.07-$24.81
