Job summary

Adult & Child seeks a Community Support Specialist to deliver community-based mental health services for children, adolescents, and adults in the Indianapolis area. You will assess, plan, and implement interventions, collaborating with families and schools to support individualized goals.

Responsibilities include coordinating care with multiple providers, documenting progress, and participating in ongoing training to strengthen youth mental health services within the organization.

Qualifications

• Knowledge of case management and rehabilitation methods and techniques related to mental health and/or substance use disorders.
• Understanding of issues faced by people with mental health or substance use disorders and how to access community resources.
• Familiarity with laws and procedures related to treating mental health and substance use disorders.
• Ability to solve problems and communicate clearly in writing and speech for reports.
• Self-motivated to deliver high performance while addressing complex client needs.
• Capability to multi-task and coordinate services from diverse community resources.

Responsibilities

• Provide community-based mental health services to clients including assessment, diagnosis, treatment planning, and intervention implementation.
• Collaborate with clients, families, and stakeholders to develop individualized treatment plans addressing client needs and goals.
• Apply strengths-based, evidence-based practices such as Motivational Interviewing and CBT/DBT techniques.
• Coordinate care with other providers including physicians, schools, and community organizations.
• Maintain accurate records of client interactions and progress in line with policy and procedures.
• Participate in staff development and quality improvement activities to enhance knowledge and skills.
