Job summary

Adult & Child Health seeks a Community Support Specialist to provide community-based mental health services for children, adolescents, and adults. You will collaborate with families to build individualized treatment plans using CBT/DBT and trauma-informed approaches.

You will coordinate care with schools, doctors, and agencies, maintain accurate records, and participate in ongoing training. A supportive team environment and a focus on recovery and resilience are offered.

Qualifications

• Bachelor's degree in psychology, social work, or related field.
• Alternative: 4-year degree with two years related experience.
• Four years of qualifying experience may also meet requirements.
• Experience delivering services to individuals with mental health or substance use disorders.
• Bilingual ability preferred.

Responsibilities

• Provide community-based mental health services to clients including assessment and treatment planning.
• Collaborate with clients and families to implement individualized plans.
• Use strengths-based approaches and evidence-based practices.
• Provide behavioral interventions and coordinate care with other providers.
• Maintain records per organizational policies and participate in training.
• Contribute to quality improvement and data reporting.
• Develop relationships with stakeholders supporting client care.
• Prepare for and facilitate PSR group sessions as relevant.
• Apply ACT/ACT TAY/IMPART knowledge where applicable.
