Job summary

Centerstone is hiring a Community Support Specialist for Indianapolis, IN to help individuals adapt to community living through personalized plans and evidence-based mental health services. You will work with children, adolescents, and adults, coordinating with families, schools, and other professionals to promote growth, stability, and independence.

Qualifications

  • Bachelor’s degree in Psychology, Social Work, or other human services field.
  • Two years of related work experience or equivalent combination.
  • Experience delivering one-on-one or group services with a rehabilitation focus.
  • Familiarity with case management, recovery, and community resources.

Responsibilities

  • Provide community-based mental health services to individuals across age groups, including assessment and treatment planning.
  • Collaborate with clients, families, schools, and other professionals to implement individualized plans.
  • Use evidence-based approaches such as MI, CBT/DBT, and trauma-informed strategies.
  • Coordinate care with primary care providers, psychiatrists, and community organizations.
  • Maintain accurate records and participate in staff development and quality initiatives.
