Job summary

Mental Health Resource Center, Inc. seeks a dedicated Registered Nurse (RN) for the FACT program. You will conduct psychiatric and health assessments, coordinate referrals, and ensure safe medication self-administration under supervision of the Psychiatrist and APRN.

The FACT team delivers community-based services to adults with severe mental illness to reduce hospitalizations and enhance independence. Most work occurs in the community with some office time; mileage reimbursement applies.

Qualifications

• Associates or Bachelor’s degree in Nursing required.
• Active Florida RN license in good standing.
• Six months of medical/surgical and/or psychiatric nursing experience.
• Proficiency with MHRC EHR and RBHS systems within 3 months.
• Valid Florida driver’s license and clean driving record.
• Ability to work across homes, offices, hospitals, and secure units.
• Strong communication and interpersonal skills.

Responsibilities

• Provides treatment, rehabilitation, and support services per FACT model.
• Maintains accurate, confidential clinical records.
• Conducts mental health assessments.
• Coordinates referrals to community providers and services.
• Coordinates referrals to community health care providers.
• Educates on mental health symptoms, medications, and recovery.
• Ensures proper medication self-administration with ARNP/Physician.
• Serves as primary case manager for assigned caseload.
