Job summary

Signature Health Inc. is seeking a Qualified Mental Health Specialist (QMHS) to operate primarily in the community, developing and executing client treatment plans and linking families to resources for comprehensive care.

You will transport clients, coordinate services, and work with a treatment team to support adults and children with mental health or addiction needs, while following safety guidelines and agency policies.

Qualifications

• High School Diploma or equivalent required; Bachelor’s Degree in related social services field preferred.
• Valid unencumbered Ohio driver’s license and proof of driver’s insurance required at all times.
• AHA Basic Life Support (BLS) certification required.
• 1–3+ years of previous case management or relevant experience preferred.
• CDCA preferred.
• Strong computer skills with Microsoft Excel, etc.
• Knowledge of the community, community organizations, and community resources.
• Knowledge of ASAM levels of care and ability to assist clients in obtaining residential, detox, and supported housing services in the community.
• Comfort delivering services primarily in community-based settings and client residences with personal vehicle transportation.

Responsibilities

• Develop, prepare, and present treatment program work material to clients.
• Plan and coordinate transportation for clients to access identified resources as part of treatment plans.
• Work hours primarily in community-based settings including client homes and other locations.
• Maintain clear communication and professional boundaries with clients.
• Serve as subject matter expert and support in developing a sober support network.
• Collaborate with client’s treatment team.
• Create an outstanding experience for adult and child clients by identifying support in education, medical, vocational, housing, and daily living needs.
