EMC Behavioral Health seeks a Qualified Mental Health Professional-Community Services (QMHP-CS) to deliver weekly skills training and tailored case management to children and families, in-person and virtual. You will design personalized treatment plans, coordinate services, and advocate for resources while collaborating with a clinical team.

Responsibilities include documenting case activity, ensuring Medicaid compliance, and maintaining knowledge of regulations.

Qualifications

• Bachelor’s degree in a healthcare-related field required.
• Current HIPAA and QMHP certification, CAN 3-5/CAN 6-17/ANSA and others as requested.
• One year in a mental health clinical setting with all age groups.
• Experience conducting assessments, diagnoses, and treatment planning.
• Strong documentation and resource navigation skills.
• Commitment to working with children with severe emotional disturbances.

Responsabilités

• Deliver weekly skills training and targeted case management tailored to each child and family.
• Develop service plans, track progress, and manage case assignments through closure.
• Navigate routine and intensive case management activities with families.
• Record case information, complete forms, and produce reports.
• Coordinate care and advocate for appropriate treatment options and resources.
• Collaborate with the clinical team to design personalized service plans.
