Job summary

Signature Health, Inc. is seeking a Qualified Mental Health Specialist (QMHS) to develop and execute client treatment plans, provide ongoing community resources, and coordinate transportation for access to services.

You will work with families, collaborate with the treatment team, and maintain electronic charting in accordance with Medicaid and CARF standards. Flexibility for telehealth and adherence to HIPAA are expected.

Qualifications

• High School Diploma or equivalent required.
• Valid unencumbered Ohio driver’s license and proof of insurance required.
• American Heart Association (AHA) Basic Life Support (BLS) certification required.
• Bachelor’s Degree in related social services field preferred.
• 1–3+ years of previous case management or relevant experience preferred.
• CDCA preferred.
• Strong computer skills with Microsoft Office, Excel, etc.
• Knowledge of the community, community organizations and resources.
• Knowledge of ASAM levels of care, ability to assist clients in obtaining residential, detox, and supported housing services.

Responsibilities

• Develop, prepare and present treatment program work material to clients.
• Plan, coordinate, and provide reliable transportation to clients for resources.
• Maintain clear communication and professional boundaries with clients.
• Act as subject matter expert and support sober network development.
• Collaborate with client’s treatment team.
• Create an outstanding experience for adult and child clients by identifying needed support areas.
• Encourage, monitor and follow up on client engagement in treatment activities.
• Serve as liaison with in-patient units.
• Maintain client charting electronically per Medicaid and CARF standards.
• Assist in crisis intervention and community outreach.
• Participate in quality assurance and utilization review activities.
• Comply with agency policies and procedures.
• Maintain HIPAA confidentiality and privacy rights.
• May perform telehealth as determined by Signature Health.
• Complete productivity/documentation requirements.
• Attend staff meetings and development programs.
• Demonstrate punctuality and attendance per policy.
