Job summary

EPIC Health Partners, LLC seeks compassionate QMHP-qualified professionals to join our Mental Health Skill Building team, delivering person-centered care in homes and community settings under the supervision of a licensed clinician. We welcome QMHP, QMHP-T, and QPPMH candidates.

This field-based role requires a valid driver’s license, CPR/First Aid certification, TB screening, and at least 1 year of behavioral health experience.

Qualifications

Bachelor's degree or higher in a Human Services-related field (e.g., Psychology, Social Work, Counseling).
At least 1 year of behavioral health experience.
Valid driver’s license and reliable transportation.
Current CPR/First Aid certification and TB screening.

Responsibilities

Deliver person-centered care in homes and community settings.
Assist in creation and revision of service plans tailored to each individual.
Monitor and ensure service plans are carried out effectively.
Provide linkage to resources and case management support for individuals.
Submit necessary documentation within established timeframes.
Maintain communication with individuals and key stakeholders for coordinated planning.
Participate in required training and team meetings.
