Signature Health seeks a Specialist, Crisis Intervention & QMHS to provide community-based behavioral health support, crisis intervention, screening, de-escalation, and care coordination for individuals in crisis.

Reporting to the Supervisor of Behavioral Health Services, the role includes developing treatment plans, arranging resources, transportation when necessary, and following up to ensure continuity of care in line with Medicaid and professional standards.

Competenze

• High School Diploma required; Bachelor's degree in a related field preferred.
• QBHS designation or PSS with at least 1 year experience in substance use and mental health; minimum 2 years in recovery recommended.
• Valid Ohio driver’s license with proof of insurance at all times.
• Minimum one year case management, crisis, peer support, or relevant behavioral health experience preferred.
• AHA/BLS certification required.
• Knowledge of local mental health system, community resources, and recovery supports.
• Basic computer skills including EHR, MS Office, and time sheet management.
• Ability to transport clients using a personal vehicle in some cases.

• Develop and implement treatment program work material and skill-building interventions for clients.
• Operate primarily in community-based settings, including client homes and other community locations.
• Identify client education, medical care, housing, and recovery support needs and coordinate resources.
• Encourage engagement in treatment plan activities and follow up as appropriate.
• Provide rapid response, de-escalation, screening, and crisis intervention in community or home settings.
• Coordinate care and link individuals to appropriate treatment services and supports.
