Job summary

Signature Health, Inc. is seeking a Qualified Mental Health Specialist (QMHS) to contribute to treatment planning and deliver ongoing community support resources for clients and families.

The role involves transportation coordination, inter-team collaboration, and ensuring HIPAA compliance within Medicaid and CARF standards. The position requires a High School Diploma or equivalent, OH driver’s license, BLS, and 1–3+ years of case management experience.

Qualifications

• High School Diploma or equivalent required.
• Valid OH driver’s license and proof of insurance required.
• AHA Basic Life Support (BLS) certification required.
• Bachelor’s Degree in related social services field preferred.
• 1–3+ years of case management or relevant experience preferred.
• CDCA preferred.
• Strong computer skills with Microsoft Excel and Office.
• Knowledge of community resources and ASAM levels of care.

Responsibilities

• Develop, prepare and present treatment program work material to clients.
• Plan and provide reliable transportation to clients to access resources.
• Maintain clear communication and professional boundaries with clients.
• Act as a subject matter expert and support sober network development.
• Collaborate with client’s treatment team.
• Maintain client charting per Medicaid/ODMHAS/CARF standards.
• Assist in crisis intervention and prevention.
• Participate in community outreach and QA activities.
• Comply with agency policies and HIPAA; may perform telehealth.
