Job summary

Signature Health, Inc. in Ohio seeks a Qualified Mental Health Specialist (QMHS) to work primarily in the community, developing and executing client treatment plans and providing ongoing support to clients and families.

The role emphasizes collaboration with the treatment team and maintaining charting compliance. As a full-time position, the QMHS delivers transportation to services, assists with accessing community resources, and supports engagement in treatment plans while adhering to agency

Qualifications

• High School Diploma or equivalent required; Bachelor’s Degree in related field preferred.
• Valid Ohio driver’s license and proof of insurance.
• AHA BLS certification required.
• 1–3+ years of case management or relevant experience preferred.
• CDCA preferred.

Responsibilities

• Develop, prepare and present treatment program work material to clients.
• Plan, coordinate and provide transportation to access resources as part of treatment plans.
• Work in community-based settings and client residences per treatment plans and policies.
• Maintain professional boundaries and clear communication with clients.
• Collaborate with treatment team to support client needs and sober networks.
• Document client progress and maintain charts per Medicaid/agency standards.
• Participate in staff meetings and quality assurance activities.
• Occasionally work evenings or extended hours as required.
