Job summary

Qualifications

• High School Diploma or equivalent required; bachelor's degree preferred.
• Strong computer skills including Microsoft Excel and office software.
• Knowledge of ASAM levels of care and community resources.

Responsibilities

• Provide high-intensity community-based behavioral health support and transport to resources as part of treatment plans.
• Coordinate and develop client treatment plans with the treatment team.
• Maintain client charts electronically per Medicaid and professional standards.
• Deliver crisis intervention, outreach, and after-hours support as needed.
• Collaborate with inpatient units and family members to support continuity of care.
