Job summary

Qualifications

• Be at least 18 years of age.
• Must be able to communicate effectively in English – both in writing and orally.
• Must have reliable transportation for transporting clients.
• Must demonstrate safe practices in all areas and exemplify positive professional standards.
• Must obtain a Class E driver’s license during new hire orientation.

Responsibilities

• Provide person-centered supports in daily living per ISP and BSP as needed.
• Offer individualized activities to promote independence and dignity.
• Input, access, and store client data using computers and devices.
• Complete required documentation, including electronic records, on shift.
• Support health and safety, including medication administration as taught.
• Attend medical appointments and coordinate with team and health coordinator.
