Job summary

Qualifications

• Must be at least 18 years old.
• Must have a high school diploma or GED.
• English language proficiency required (written and verbal).
• Must have reliable transportation.
• Must obtain a Class E driver's license during new hire orientation.
• Must not have a disqualifying felony and must not have substantiated abuse or neglect.

Responsibilities

• Provides high quality, person-centered daily living supports.
• Follows Individual Support Plans (ISP) and behavioral support plans (BSP).
• Documents in ISP and maintains required records according to policy.
• Attends medical appointments and coordinates with health team as needed.
• Ensures health and safety guidelines are followed at all times.
