Job summary

Enriching Lives is seeking a Direct Support Provider to assist adults with developmental disabilities in achieving their personal goals and participating in community activities. You will support daily living tasks, transportation, personal care, and skill-building aligned with each Individual Service Plan.

Strong communication, reliability, and compassion are essential. You will maintain progress notes, liaise with families and agencies, and respond to needs with appropriate care and safety.

Qualifications

• Ability to read and comprehend simple instructions and memos.
• Ability to write simple correspondence and complete documentation.
• Strong communication and interpersonal skills.
• Patience, empathy, and reliability in support settings.

Responsibilities

• Assist Individuals with transportation to activities and appointments.
• Provide personal care and life-skills support.
• Document progress notes and required reports in a timely manner.
• Facilitate community participation and relationship development as per Individual Service Plan.
• Coordinate with families, providers, and regulatory bodies as needed.

Education
