Job summary

The Community Liaison role at Caretech is a full-time, on-site position based in the TriCities market. You will build and maintain strong relationships with seniors, families, referral partners, and local organizations to promote Caretech’s home care services.

You will conduct outreach at local events, visit hospitals and clinics, meet social workers and case managers, and guide families on care options, transitions from hospital to home, and Medicaid Waiver Programs.

The organization provides a full range of care, including companion care, personal care, dementia care, and respite care, helping clients transition smoothly from hospital to home. Caretech also assists families in understanding and accessing Medicaid Waiver Programs, including options for paid family caregiving. Serving communities across Nebraska, including Omaha, Lincoln, Bellevue, and many other cities, Caretech focuses on comfort, dignity, and peace of mind for the clients and caregivers it supports.

Role Description The Community Liaison is a full-time, on-site role based in TriCities market responsible for building and maintaining strong relationships with seniors, families, referral partners, and local organizations. This role includes conducting community outreach activities, such as attending local events, visiting hospitals and clinics, and meeting with social workers and case managers to introduce Caretech's services. The Community Liaison will provide information and guidance to families on home care options, support the transition from hospital to home, and help explain Medicaid Waiver Programs and related resources. Daily tasks involve responding to inquiries, delivering high-quality customer service, coordinating with the care team to ensure smooth onboarding of clients, and representing Caretech's values and standards in the community.

Qualifications

  • Community Engagement and Community Outreach skills to build partnerships and increase awareness of Caretech's services.
  • Strong Communication and Customer Service skills to support seniors, families, and referral sources with clarity and empathy.
  • Understanding of Home Care services and the ability to explain care options, processes, and available support programs.
  • Experience working with diverse communities and comfort interacting with individuals from various cultural and socioeconomic backgrounds.
  • Organizational and time management skills, with the ability to prioritize multiple outreach and client support activities.
  • Knowledge of local healthcare and social services networks in Omaha and broader Nebraska is preferred.
  • Previous experience in community relations, social services, healthcare, or senior care is beneficial.
  • High school diploma or equivalent required; further education in social work, communications, healthcare, or a related field is a plus.
