Job summary

ServiceSource Inc. in Clearfield, UT, is seeking a part-time Rehabilitation Support Specialist.

The role provides support to individuals in PARC’s Community Integration Services, focusing on safety, well-being, health, recreation, and independence skill development. Responsibilities include assisting with personal care, medications, and behavior supports, documenting progress, and facilitating activities that promote inclusion and community participation.

Qualifications

• High school diploma or GED required.
• One year experience working with adults with disabilities preferred.
• Valid driver’s license and/or access to reliable transportation for travel required.
• Ability to pass and maintain Department of Human Services background clearance required.
• Knowledge of Microsoft Office and basic computer literacy.

Responsibilities

• Provide support and training to small groups of persons served to ensure well-being, safety, and needs are met.
• Assist with personal care, mobility, positive behavior interventions, and self-administration of medications.
• Help participants identify and achieve goals and participate in activities to increase independence and inclusion.
• Document progress daily including evaluations, assessments, support plans, and incident reports.
• Support program functions: skills development classes, community activities, and transportation.
