ServiceSource is seeking a dedicated professional to lead and support participants in daily programming, including academic, social, life and leisure skills. You will assist with Personal Centered Plans (PCP) and ensure compliance with Medicaid, Licensure and CARF guidelines.

Responsibilities include active engagement, documentation, and communication with families and coordinators, while maintaining safe, healthy environments and accurate records. Training and growth opportunities are offered.

Formación

• High School diploma or GED with 1 year of experience in working with adults with developmental disabilities.
• Preferred: college coursework in human services or a bachelor’s degree in human services and related experience.

Responsabilidades

• Provide active engagement and support for participants including community inclusion, training, leisure, communication, mobility and personal care.
• Develop and implement Person Centered Plans (PCP) outcomes and documentation to ensure safe, healthy progress.
• Maintain accurate PCP records, quarterly reports and daily notes per regulatory and company standards.
• Communicate with families, guardians and coordinators about illness, incidents and progress.
• Follow written protocols, report unsafe conditions, and complete incident reports as required.
