Job summary

ServiceSource is seeking a dedicated support professional to assist PARC’s Community Integration Services program participants in Utah. The role focuses on safety, well-being, and skill development across health, recreation, social development, and independence initiatives.

You will work with individuals to implement goals from their Individual Support Plans, provide group and one-on-one supports, and document progress daily in compliance with program guidelines and funding requirements.

Qualifications

• High school diploma or GED required.
• One year experience with adults with disabilities preferred.
• Valid driver’s license or access to reliable transportation for work-related travel.
• Ability to pass and maintain Department of Human Services background clearance required.

Responsibilities

• Provide support and training to small groups to ensure well-being, safety, and needs while following program guidelines.
• Support and assist participants with personal care, mobility, and medication self-administration.
• Help participants identify and achieve goals from their Individual Support Plans and increase independence.
• Document progress daily including evaluations, assessments, support strategies, and incident reports.
• Participate in program activities and monitor safety during meals, breaks, and recreation.
