Job summary

ServiceSource Inc. seeks a Part-time Program Assistant – Job Coach in the Clearfield/Bountiful area to support participants with disabilities in achieving successful employment.

Under the supervision of the Director of Rehab, you will supervise, train, and assist participants in developing job tasks and independent work routines. You will help with accommodations, transportation, and social integration, maintain accurate records including IPP/IPIEP notes, and collaborate with employers and

Qualifications

• High school diploma or GED required.
• Valid driver’s license and reliable transportation.
• Previous experience supporting adults with disabilities preferred.
• Flexibility to work outside normal hours, including evenings and weekends, is highly preferred.
• Pass and maintain Department of Human Services background clearance is required.

Responsibilities

• Oversee participants in the efficiency of work performed and ensure quality control of assigned jobs.
• Assist participants in acquiring skills to perform tasks independently, including behavioral supports.
• Help participants locate and secure transportation to and from work, and support social and personal skill development.
• Produce monthly billing reports, participant reports, case notes, and other supporting documentation.
• Develop positive relationships with employers, community agencies, and outside vendors.
• Serve as a member of the IPP/IEP team and document progress; evaluate participants and gather baseline data.
