Job summary

CareLink Community Support Services is seeking a role that supports individuals with disabilities by assisting with daily living activities and promoting recovery, wellness, and independence. Responsibilities include documenting progress and implementing person-centered activities to foster community integration.

The position involves assisting with meal prep, mobility, medication administration, and maintaining a safe, welcoming environment while collaborating with a dedicated team to uphold

Qualifications

• Knowledge of Microsoft Office products.
• Excellent communication and interpersonal skills.
• Ability to multitask and be flexible with regard to workload and assignments.
• Must be able to write and speak English clearly.
• Ability to drive agency vehicles is required.

Responsibilities

• Participate in the development and implementation of service/recovery plans reflecting the individual’s preferences, goals, lifestyles, and interests.
• Assess, teach, evaluate, and supervise individual’s skills in daily living, meal planning, housekeeping, career needs, and social, recreational, and community activities.
• Complete written documentation daily reflecting the individual’s recovery and progress on goals and staff interventions.
• Assist and oversee individuals with medication regimen including PRN medications.
• Engage in productive and therapeutic activities.
• Encourage and support individuals in developing positive relationships with family and significant others.
• Assist with maintaining cleanliness and orderliness of individual rooms and residence.
• Assist with meal preparation and dietary considerations.
• Access community medical resources as applicable.
• Participate in shift activities (cleaning, trash removal, linens, bathrooms, common areas).
• Maintain cleanliness of work areas and medication storage.
