Job summary

Entrust Community Services is seeking a diligent Case Aide to welcome participants, assist with intake, and connect individuals to appropriate resources. You will support Case Managers and maintain accurate records to ensure smooth service delivery.

The role emphasizes strong communication, organizational skills, and the ability to work both independently and within a team in an office-based environment.

Qualifications

• High school diploma or GED required; additional college-level or specialized training preferred.
• Undergraduate degree in psychology, sociology, special education, or related field preferred.
• At least one year of human services experience.

Responsibilities

• Welcome visitors and connect them with the appropriate staff or services.
• Answer incoming calls, route calls, and take messages.
• Conduct initial eligibility screenings and help establish new participant files.
• Schedule intake appointments for new participants with Case Managers.
• Assist Case Managers with new participant intake activities and Job Foundations.
• Maintain accurate participant documentation and records.
• Scan, file, organize, and upload documents into the SETWorks database.
• Research community resources and refer to outside agencies as appropriate.
• Develop and maintain professional relationships with community partners and care providers.
• Provide coverage and support for Case Managers, Placement Specialists, and Community Support Specialists as needed.
• Assist with community outreach activities.
• Help maintain office supplies and coordinate supply requests.
• Complete other duties as assigned.
