Job summary

Melwood Horticultural Training Center Inc. seeks a Community Connector to support individuals in a community-based day services model.

The role focuses on promoting independence, integrating participants into community activities, and building personalized daily schedules aligned with each person’s desires and needs. Key responsibilities include implementing Individual Plans, engaging individuals in meaningful activities, documenting progress, and collaborating with the interdisciplinary team.

Qualifications

• High School diploma or equivalent required.
• Three years experience working with people with disabilities or other behavioral issues.
• Bachelor's degree in special education, therapeutic recreation, psychology, or related field preferred.

Responsibilities

• Work with assigned individuals to implement their Individual Plan, integrate them into the community, build skills, and develop individualized goal-oriented daily schedules.
• Engage person served in meaningful community activities to develop skills and social supports for positive personal growth.
• Provide services as outlined in Individual Plans, ensuring goals are appropriate, implemented, and progress benchmarks are met.
• Demonstrate positive behavioral management techniques in a person-centered, age-appropriate manner.
• Complete progress notes, daily logs, timesheets, mileage logs, and other documentation thoroughly and promptly.
• Facilitate an individual's self-esteem through positive interactions and building trust.
• Develop activities that promote community participation, self-help employment, and independent living.
• Communicate with supervisor and team regarding schedule implementation.
• Attend and participate in required training, staff meetings, stakeholder meetings, and supervision meetings.
• Other duties as assigned.

Education
