The role involves regular meetings with consumers, collaborative teamwork, and potential driving responsibilities to support services across Wayne County. A valid driver’s license is required.

Qualifications

• Strong communication and interpersonal skills are essential.
• Experience in human services and case management is preferred.
• Knowledge of coordinating with community resources is beneficial.

Responsabilités

• Case management and assessment of consumers assigned by Case Management Supervisor.
• Provide on-site case management services as needed in Wayne County Schools.
• Maintain complete records for all consumers and complete intake procedures.
