Job summary

Goodwill Industries of Wayne & Holmes Co., Inc. seeks a CCMEP Case Manager in Ohio to assess and manage consumer progress, maintain records, and coordinate intake and ongoing services within the CCMEP program.

You will collaborate with the Case Management Supervisor and support individuals toward self-sufficiency. The role requires strong communication, organization, and the ability to work independently or with a team; CPR/First Aid certification can be obtained after hire.

Qualifications

  • Education: College degree preferred, but not required (HS diploma with relevant experience accepted)
  • Experience: 3 years in related field preferred
  • Certification: CPR/First Aid (can be obtained upon hire)
  • Other: Valid driver’s license and insurability or reliable transportation

Responsibilities

  • Case management and assessment of consumers assigned by Case Management Supervisor.
  • Maintain records on all consumers receiving services.
  • Performs intake for consumers and conducts weekly progress reviews.
  • Compiles reports and provides updates to supervisor.
  • Interacts with consumers and assists on rehab process and service planning.

CCMEP Case Manager

ESSENTIAL FUNCTIONS:

  • Case Management and assessment of consumers assigned by Case Management Supervisor. This includes CCMEP.
  • May provide on-site case management services throughout Wayne County Schools
  • Maintains complete records on all consumers receiving services as assigned
  • Performs intake for consumers
  • Meets with consumers on a weekly basis to review progress on service plans and updates progress in the Consumer Database
  • Attends meetings and case staffings as required for each consumer
  • Initiates, maintains and documents service provided for consumers and updates in the consumer database
  • Prepares required reports on all consumers and trainees
  • Provides progress reports for the Case Management Supervisor on a weekly basis for everyone served.
  • Works directly with consumers and trainees in all phases of the rehabilitation process
  • Provides retention services to consumers
  • May function as a driver when needed
  • Performs other duties as assigned by the Case Management Supervisor and the Mission Services Director

  • Communicates clearly and effectively, both verbally and written with all staff
  • Complies with all established work rules, policies, procedures and safety rules
  • Represents the organization in a professional and courteous manner
  • Is able to educate stakeholders and community about the entire agency

  • Other:Valid driver’s license and insurability or ability to provide reliable transportation
