Job summary

Old Colony Elder Services, Inc. in Brockton is expanding its Housing Team as we grow to 11 more towns along the South Shore. The Housing Specialist will assess needs on-site and link residents to community programs and services.

You will lead a multidisciplinary assessment team, coordinate care conferences with residents and families, and plan social, educational, and recreational activities. We offer a collaborative, compassionate work culture with flexible hybrid work options and opportunities

Qualifications

• Experience in human services, preferably with the elder population.
• Bachelor's degree in social work or related field preferred.
• Excellent written and verbal communication.
• Excellent computer skills and MS Office proficiency.

We are expanding to 11 more towns covering the South Shore area, including Quincy, Braintree, Weymouth, etc.

OCES supports older adults and individuals with disabilities by providing vital information and coordination of services. By promoting healthy, safe living for our consumers, we hope to allow them to stay active and engaged in their lives and in their own communities for as long as possible.

We are seeking a dedicated, caring Housing Specialist professional who enjoys working in the community with our Housing team to ensure services and support to residents on-site in our Brockton area locations.

What you'll do:

• Review the needs of residents to link the community programs and services

What you'll bring:

• Your passion for helping others and desire to make a difference.
• Adaptability to be able to change as the needs of consumers and/or programs change
• At least two years of experience in the human services field, preferably with the elder population
• Bachelor's degree in social work or related field preferred
• Excellent written and verbal skills.
• Excellent computer skills.
• Proficient with Microsoft Office Suite or related software.
• Bilingual a plus; Cantonese, Haitian Creole or Cape Verde Creole is preferred
