Old Colony Elder Services in Plymouth is seeking a dedicated Housing Specialist to support older adults and individuals with disabilities in the South Shore region. The role focuses on linking residents to community programs, coordinating care, and promoting safe, independent living within on-site locations.

You will lead multidisciplinary teams, plan activities, and prepare communications to keep residents informed.

Formación

• Two+ years in human services, preferably with elder population.
• Bachelor's degree in social work or related field preferred.
• Excellent written and verbal communication skills.
• Proficiency with Microsoft Office or related software.

Responsabilidades

• Review residents' needs to link community programs and services.
• Lead a multidisciplinary assessment team and coordinate service.
• Facilitate care conferences with residents, family, and others.
• Plan and coordinate social, educational, and recreational activities.
• Prepare and distribute a newsletter to keep residents informed of planned activities.
