Job summary

Harbor Health's Elder Service Plan in Brockton, MA seeks a Housing Support & Resource Specialist to help participants and families access housing resources, SNAP, and other services. The HSRS collaborates with social workers and housing partners to address social determinants of health and support stable housing outcomes.

The role requires a BA in social work or related field (LSW preferred), BLS certification, strong communication, and willingness to travel across Mattapan/Brockton housing

Qualifications

• Bachelor’s degree in Social Work (preferred) or related field; may substitute 4 years of related experience for the degree.
• Licenses: LSW preferred; BLS certification required.

Responsibilities

• Establishes trusting relationships with ESP participants and caregivers; provides general support and advocates for health needs.
• Identifies barriers to stable housing and basic needs; provides referrals to local, state, and federal programs.
• Assists participants in obtaining documents and completing applications for housing, SNAP, utilities, and MassHealth redeterminations.
• Coordinates with ESP social workers and care teams to ensure coordinated care for participants.
