Job summary

Mercy is seeking a Community Health Worker to serve as a trusted liaison between health and social services and the community. The role focuses on facilitating access to care, improving service delivery quality, and upholding Mercy standards and mission.

The ideal candidate has 1–3 years of CHW experience, a high school diploma or equivalent, completion of Mercy’s CHW training within 6–12 months, Basic Life Support, and strong relationship and communication skills.

This trusting relationship enables the worker to serve as a liaison/link between health/social services and the community to facilitate access to services and improve the quality and cultural competence of service delivery. Experience:

1-3 years CHW experience or equivalent

Certification/Registration:

Mercy provided Community Health Worker training certificate within 6-12 months
