Job summary

Mercy St. Francis Hospital in Mountain View, Missouri is seeking a full-time Community Health Worker to act as a trusted liaison between health and social services and the community.

You will help individuals access services, document activity in the EMR, and build strong community relationships. Ideal candidates have 1–3 years CHW experience (or equivalent), a High School Diploma, and the ability to work independently with compassion for underserved residents.

Qualifications

• High School Diploma or equivalent required.
• Mercy-provided Community Health Worker training within 6–12 months.
• Basic Life Support certification.
• One to three years CHW experience or equivalent.

Responsibilities

• Act as a liaison between health/social services and the community to facilitate access to services.
• Develop trusting relationships with community members.
• Document activities in the EMR and use Microsoft applications.
• Work independently to support underserved populations.
• Collaborate with teams to improve service quality and cultural competence.

Minimum Required Qualifications

Education:

High School Diploma or equivalent

Licensure:

None

Experience:

1-3 years CHW experience or equivalent

Certification/Registration:

Mercy provided Community Health Worker training certificate within 6-12 months

Strong relationship and collaboration skills.
