Job summary

Mercy is seeking a Community Health Worker to serve as a trusted liaison between health and social services and the community. The role focuses on facilitating access to care, improving service delivery quality, and upholding Mercy standards and mission.

The ideal candidate has 1–3 years of CHW experience, a high school diploma or equivalent, completion of Mercy’s CHW training within 6–12 months, Basic Life Support, and strong relationship and communication skills.

Qualifications

• High School Diploma or equivalent.
• 1-3 years CHW experience or equivalent.
• Mercy-provided Community Health Worker training certificate within 6-12 months.
• Basic Life Support.
• Must be proficient with Microsoft Applications and able to document in an EMR.
• Must have access to reliable transportation.
