Your Health Organization in Florence, SC is seeking a dedicated Community Health Worker to serve patients in homes and care facilities. You will support care teams, arrange visits, and help patients access medications and DME, with daily travel required.

The role demands strong communication skills, experience in home health, and the ability to coordinate telehealth visits and RPM setup for patient support. A valid driver's license and reliable vehicle are essential.

Qualifications

• Certified Medical Assistant, Paramedic, or Licensed Nurse preferred.
• Phlebotomy experience preferred; training available if not experienced.
• Experience in public health or home health preferred.
• Experience with DME delivery or logistics operations preferred.
• Must be proficient with technology tools for RPM, telemedicine, and EMR.
• Strong organizational and time management skills.
• Ability to read and communicate effectively.
• Basic computer knowledge.
• Must work independently with minimal supervision.
• Must have good interpersonal and communication skills.
• Must be able to work flexible hours and travel between offices.
• Must be a licensed driver with insured vehicle in good condition.
• Clean driving record and professional vehicle operation experience.

Responsibilities

• Work closely with care team to ensure coordinated patient care.
• Coordinate visits with PTs/OTs and Providers in ALFs and homes.
• Deliver medications to patients' homes and perform medication reconciliation.
• Coordinate telehealth visits between patients and pharmacists, specialists, and PCPs.
• Set up Remote Patient Monitoring (RPM) training and provide ongoing support and troubleshooting.
• Deliver durable medical equipment (DME) to patients and provide training on correct use.
• Perform routine venipuncture for laboratory testing and proper specimen handling.
• Collect blood specimens during home or facility visits.
