Job summary
Your Health in Florence, SC is hiring a dedicated Community Health Worker (CHW) to serve patients in the Florence area. The CHW links patients with healthcare services, visits homes and ALFs, facilitates visits with providers, and delivers medications and DME to ensure timely access to essentials.
Travel is required daily with reliable transportation. This full-time role operates on 12-hour shifts from 7:00 AM to 7:00 PM, covering Florence and surrounding areas as part of our physician group
Qualifications
- Certified Medical Assistant, Paramedic, or Licensed Nurse preferred.
- Phlebotomy experience preferred; willing to train if not experienced.
- Public health or home health experience preferred.
- Experience with DME delivery or logistics operations preferred.
- Proficient with technology tools for RPM, telemedicine, and EMRs.
- Strong organizational, time management, and communication skills.
- Must be able to travel between offices and facilities daily.
Responsibilities
- Work with care team to ensure coordinated patient care per plan.
- Deliver medications to patients' homes and review instructions.
- Coordinate and facilitate telehealth visits with pharmacists, specialists, and providers.
- Set up RPM equipment training and provide ongoing support and troubleshooting.
- Deliver DME to homes and ALFs and train patients/caregivers on use.
- Perform venipuncture and collect blood specimens as needed.
- Travel between offices, facilities, and patient homes daily.
