Job summary

WelbeHealth seeks a proactive Outreach and Community Engagement Lead to coordinate referrals and enrollment activities in Modesto, CA. You will build partnerships with physicians, hospitals, post-acute facilities, and community organizations to drive monthly enrollment goals.

You will manage education for referral sources, ensure compliant marketing practices, document work in Salesforce and MS Office, and align with the MOEE Playbook while maintaining confidentiality and quality improvement

Responsibilities

• Coordinate and execute outreach, PR and community engagement activities to meet enrollment goals.
• Identify opportunities to collaborate with community organizations reflecting mission.
• Establish relationships with community leaders and partners.
• Develop and implement annual marketing plan to achieve enrollment goals.
• Provide education to referral sources about WelbeHealth services.
• Comply with marketing rules and ensure privacy.
• Complete required documentation and generate reports in Salesforce, Athena and MS Office Suite.
• Follow WelbeHealth policies and participate in training and quality improvement activities.
• Communicate workload and priorities with Supervisor.
• Complete mandated trainings and health screenings.
• Willingness to work varied schedule including evenings and weekends.
• Other duties as assigned.
