Job summary

WelbeHealth is seeking a field-based Community Outreach Coordinator in California to drive outreach, enrollments and referral partnerships. You will collaborate with community organizations, physicians and hospitals to meet monthly enrollment goals and reflect our mission.

You will coordinate events, provide education about WelbeHealth services, maintain relationships with referral sources, and ensure compliant marketing practices while documenting activities in Salesforce and MS Office.

Qualifications

• Bilingual Spanish required for outreach and education.
• Experience in B2C sales and health services is preferred.
• Knowledge of community-based outreach and referral development.

Job description

• Coordinate and execute outreach, PR and community engagement activities to ensure consistent achievement of monthly referral, home visit and enrollment goals.
• Identify opportunities to collaborate with community organizations and execute against those opportunities in a thoughtful manner that reflects our mission and value proposition.
• Establish and maintain best in class relationships with community leaders and partners including physicians, hospital discharge planners, post-acute facilities and other potential referral sources in the service area.
• Develop and implement annual marketing plan to achieve monthly enrollment goals with MOEE Director.
• Provide education to referral source in the community regarding WelbeHealth services.
• Demonstrate in-depth knowledge of all relevant components of MOEE Playbook.
• Comply with all rules pertaining to allowable and prohibited marketing practices.
• Identify multiple potential opportunities for process improvement.
• Must be willing and have the ability to work a varied schedule that may include evening nights, weekends and overtime.
• Complete all required documentation and generate reports in a timely and accurate manner, including in Salesforce, Athena and MS Office Suite.
• Protect privacy and maintain confidentiality of all company procedures and information about team members, participants, and families.
• Follow WelbeHealth policies and procedures and participate in any required Quality Improvement activities, staff training and meetings.
• Communicate regularly with Supervisor and team regarding workload and priorities.
• Timely completion of all mandated trainings and education.
• Timely completion of all mandated occupational health screenings as needed.
• Exercises flexibility in performing assignments as business needs evolve.
• Other duties as assigned.
