Job summary

Qualifications

• Certified Application Counselor or Navigator required.

Responsibilities

• Coordinate social assistance referrals with care teams.
• Assist patients in enrollment for health insurance.
• Provide services in the language most comfortable for the patient.
• Document referrals and patient communications in the EHR.
• Develop relationships with local organizations for referrals.
• Represent Union Community Care in public venues.
